Application Forms
On this page you will find application forms to become a Crosslink affiliated church and an endorsed minister. Each affiliated church and endorsed minister must nominate 2 apostolic mentors as a part of their application. If you are applying to join Crosslink, please ensure those nominated as apostolic mentors have access to the two documents “Apostolic Relationships” and “Apostolic Functions".
